Planning your activity ahead of time means you are more likely to Find Thirty® every day. So to enter the weekly draw, you'll need to plan and fill in your physical activity for the week on the planner included on the entry form.
Here's what you need to do:
- Download and print the entry form for the relevant week below, and start planning your activity!
- Complete the entry form, remembering to fill in your name, phone number and postcode.
- Post your completed entry form to: Find Thirty® Promotion, PO Box 1133, Subiaco WA 6904.
